1. Open Enrollment – The open enrollment period for the Porterville Unified Health & Welfare Plan is designated as the four-week period from September 15th to October 15th. During this period of time, all eligible employees may make any changes desired in their coverage under the Employee Benefits Program. The changes will become effective the following January 1st.
2. Newborn Baby – Remember to bring in the form from the hospital to enroll your newborn within the first 30 days (This has child’s name, date of birth, parent’s names, etc.). If you miss this 30 day deadline, you will not be able to enroll your newborn child until the next open enrollment. After you receive the real birth certificate and Social Security card – we will need copies to include in the file.
3. Newlywed - You have 30 days from the date of marriage to enroll a new spouse. Bring in the marriage certificate and a copy of your spouse’s Social Security card and other insurance if applicable. If you miss this deadline you will have to wait until the next open enrollment period.
